Community Physiotherapy Team
Community rehabilitation covers a range of services that promote faster recovery from illness, prevent hospital admission and maximise independent living. The adult therapy rehabilitation team specialises in elderly rehabilitation, orthopaedic conditions and respiratory therapy.

Contact a Family is a national charity for families with disabled children. In Southwark, they offer support to parents and carers of disabled children as well as professionals working with families.
Cool 2 Care
A childcare introductory service that recruits, screens and trains care-workers for families with disabled children or young adults. Operating in 20 different areas around the UK, Cool2Care is both a recruitment and care organisation with a difference.
Deafblind UK
Deafblind UK knows that with the right support, deafblind people can live healthier, enjoyable and dignified lives. We are open, accessible and relevant to all deafblind people across England, Wales and Northern Ireland – irrelevant of age, gender and background – offering crucial support including guidance, understanding, emotional support and opportunities that few other agencies can supply.
DeafHope
DeafHope is the only sign-language based service designed to help Deaf women and children who suffer domestic violence. Deaf women are more at risk of domestic abuse than hearing women. When sign language is the main way you communicate it can be very hard to get help. If you are being abused, the DeafHope team are here to help you.
Dial a Ride
If your disabled and can't use public transport try Dial a Ride. Its free, door to door and great for getting out and about. Only members can use Dial a Ride, you must have a permanent or long term disability which means you are unable to use public transport some or all of the time.

Disability Rights UK works to create a society where everyone with lived experience of disability or health conditions can participate equally as full citizens, to strengthen the voice of disabled people to make our rights real, as an effective national organisation led by people with a wide range of impairments or health conditions.

H.A.D. Support
H.A.D. Support are a self-help peer support group. The members of the group come from a wide cross-section of backgrounds but they have two things in common; they are all living with HIV and they are also living with Diabetes.
HAIL LTD
HAIL’s mission is to support people with a learning disability and other vulnerable people to lead their own lives. HAIL believes everyone has a right to have good support to make sure their needs are met so life is a little easier. At HAIL we want to see people learning the things they want to know, be it how to make a cup of tea, which bus to take to get to their friend’s house, when to drop a stitch or how to start a business. HAIL wants to see people work with their friends, families and partners to find ways they can spend more good times together and less time with the people that are paid to be around. HAIL supports people to find things they want to do and people who they would like to do them with.
