RECONNECT Service

RECONNECT (part of the Community L&D offer) commissioned in 2021 is an NHSE funded service delivered in HMP Brixton and HMP Bronzefied, the service supports adults who will be residing in London post upon release.

The RECONNECT project is supported by Community Link Workers and Peer Supporters and aims to provide practical and emotional support to people in the lead up to, and post release from custody, to ensure their health and social care needs are met. There is a focus on reconnecting people with health services in the community and supporting people to make and attend appointments.
