NIHCSS Community Support

Our community outreach team will work with a client to support their on-going recovery.    Referrals are received through Social Care.  We concentrate on the goals and outcomes of that individual and work with them in a person centred way to support what they want to achieve as well as what is important in their life.  Some of the areas that we can support with are:

• Daily living tasks such as supporting with meal planning, preparing meals and shopping
• Accessing the community including using and building confidence on using public transport
• Supporting to look at activities such as accessing voluntary work, groups or other areas of interest
• Engaging with and attending medical appointments
• Debts, managing bills, understanding benefits and finances

We as an organisation are committed to promoting the independence and growth of individuals and support in their journey of recovery.
