Services offered:
Church Hill is part of Together for Mental Wellbeing’s North Hampshire Housing and Support Services, offering support to adults living with mental health conditions.
The service offers supported accommodation to people needing intensive input and provides up to 18 hours of support per week, to each of the five individuals living there.
Our staff are available onsite to offer support at several points throughout the day or week, including weekends. On-call support is also available if necessary.
The aim of the service is to help each individual to move forward with their life and increase the levels of independence, so that they move onto the next stage or into independent living within a relatively short space of time.
We offer flexible one-to-one support and we will match each individual with a keyworker, who will work alongside the person to help them decide on goals and ways to achieve them. We aim to support each individual to build their strengths and develop daily living skills, as well as tools to help manage life and mental health as independently as they can.
We support each individual to manage their tenancy and find move-on accommodation, by also encouraging people to develop their skills, train, access volunteering opportunities and employment.
Peer support is also available at Church Hill through our partner in delivering this service, Sanctuary Supported Living.
We have designed group sessions to support further development of skills and abilities and continue to inform the format of the groups depending on need. Some typical groups include: managing your home, budgeting, confidence building, managing relationships, etc.
We accept referrals from or on behalf of North Hampshire residents aged 18+ years with eligible mental health related social care needs (as per Care Act 2014 eligibility criteria).
