NHS Health Checks

The Health Improvement Hub is Southwark’s lifestyle support service, helping people who would like support on weight management and how to become more physically active. The Hub is a FREE service offered to people who live in Southwark (or are registered with a Southwark GP) who have had an NHS Health Check and are found to have one or more of the following:

A body mass index of 30 or over

A risk of 20% or more of developing heart disease, high blood pressure, stroke, type 2 diabetes and kidney disease (as identified in the Health Check)

High sugar levels

Support includes one-to-one motivational interviewing, referral to Shape Up and Walking Away from Diabetes courses.

You can have an NHS Health Check by:
• contacting your GP
• making an appointment at a participating community pharmacy (information on website)
• with our outreach team who work from shopping centres, libraries and other community venues across Southwark (information on dates and venues on website)
